Hello Neighbor is seeking an Intensive Case Manager who will support the efforts of the Family Services Team as they welcome Pittsburgh’s newest neighbors.

The Intensive Case Manager is responsible for providing case management services to newly arrived refugees, SIV’s, and Afghan parolees. The case management goals are individualized in nature and span across various aspects of individual, familial, and/or household self-sufficiency. This includes but is not limited to physical healthcare navigation, mental health resource attainment, school-related issues, and others. This position aims to address issues with a lens of trauma-informed care and focuses on assisting clients in attaining self-sufficiency. This position will include working internally with other Hello Neighbor programs as well as external facilitation of care coordination teams.

Founded in 2017, Hello Neighbor works to improve the lives of recently resettled refugee families by matching them with dedicated neighbors to guide and support them in their new lives. 2023 marks our 5th year of operation and our first with a $3 million budget. We have an ambitious year of anniversary activations planned and are excited to post this newly created position to join the team and hit the ground running.

This is an in-house position at our Pittsburgh, PA office in Larimer. We are currently on a hybrid return-to-work and in the office on Tuesdays, Wednesdays, and Thursdays. We require COVID-19 vaccination and a booster for full-time employees.

Reports To: Intensive Support Program Manager 

Key Responsibilities:

  • Conduct intakes, and assessments, and build comprehensive service plans for clients and/or families
  • Provide direct and indirect case management services based on service plans
  • Facilitate care coordination teams to assist clients in navigating physical healthcare systems as necessary 
  • Initiate referrals to necessary community-based services as needs as identified
  • Advocate for appropriate use of interpretation services throughout the continuum of care and internal or external services including while accessing medical and other social service systems 
  • Assist with crisis situations as they arise (in collaboration with your team) 
  • Complete and maintain case documentation in an efficient and accurate manner
  • Provide extended cultural orientation across services 
  • Provide transportation resource navigation including accessing MATP and/or bus training
  • Help clients meet self-sufficiency goals as soon as appropriate and possible given vulnerabilities
  • Provide timely, detailed case notes for case management services 
  • Maintain detailed relationship records for all Hello Neighbor partners

Preferred Qualifications:

  • Requires 4+ years of related and/or applicable work experience and a demonstrated ability to effectively work with refugee and immigrant communities
  • Bachelor’s Degree
  • Willingness to work in a fast-paced, multicultural environment
  • Car, valid driver’s license, and proof of insurance
  • Alignment with and support of Hello Neighbor’s core values
  • Flexibility and ability to work nights and weekends as needed
  • Demonstrated ability to effectively work with refugee & immigrant communities & utilize interpretation appropriately
  • Willingness to work in a fast-paced, multicultural environment
  • Car, valid driver’s license, & proof of insurance
  • Alignment with and support of Hello Neighbor’s core values
  • Flexibility and ability to work nights and weekends as needed
  • Digital literacy and the ability to learn new software
  • Knowledge of social service and healthcare systems or any medical field experience is a significant plus
  • Ability and willingness to be on call for entire weekends on a rotating schedule with your team
